e 15% de las mujeres tratadas se diagnostican nuevamente de una lesión
intraepitelial post-tratamiento en los siguientes 2 años
e Lesión persistente: lesión incompletamente tratada, que se detecta en los
controles durante el primer año
e Lesión recurrente: en el seguimiento posterior al año
e Riesgo de cáncer cervical entre las mujeres tratadas de CIN 3 es 12 veces
mayor que el de la población general durante los siguientes 10 años
e Objetivo del seguimiento post-tratamiento: diagnosticar precozmente la
persistencia o recidiva de la lesión y evitar su progresión a cáncer
PERSISTENCIA DEL DE HPV POST-CONO
- Mujeres con elevado riesgo de persistencia del VPH (mayores de 50 años con
lesiones extensas y/o márgenes afectos o inmunosuprimidas) presentan un alto
riesgo de recurrencia.
- VPH-AR, no solo necesario para el desarrollo de lesiones de alto grado, sino
también para el desarrollo de las segundas lesiones tras el tratamiento
- La escisión total de la lesión consigue la negativización del VPH en
aproximadamente el 70%
- VPH-AR positivo: a los 6-12 meses postratamiento, permite identificar con una
elevada sensibilidad las pacientes con fallo del tratamiento
- VPH negativo: riesgo extremadamente bajo de recurrencia (valor predictivo
negativo próximo al 100%)

Persistencia de HPV post-cono
- Mujeres con elevado riesgo de persistencia del VPH (mayores de 50 años con
lesiones extensas y/o márgenes afectos o inmunosuprimidas) presentan un alto
riesgo de recurrencia.
- Infección por VPH-AR, no es solo necesaria para el desarrollo de lesiones de
alto grado, sino también para el desarrollo de las segundas lesiones tras el
tratamiento.
- Un tratamiento efectivo elimina tanto la lesión premaligna como el VPH-AR
causante
- Las mujeres que desarrollan una lesión persistente/recurrente, la infección por
VPH-AR también persiste después de tratamiento

Conclusion(s): Endocervicoscopy appears to be a safe and effective office technique, improving the diagnostic
work-up of cervical intraepithelial lesions. The precise localization of the lesions allows for the depth of cone
excision to be tailored, thus leading to a more conservative treatment and preserving the future fertility of
